The Hennepin County Health Care for the Homeless Project is seeking three part-time Family or Adult Nurse Practitioners who have an interest in, and skill and experience working with, underserved populations. These positions will provide medical services at local shelters and drop-in centers serving homeless clients in the downtown Minneapolis area.

The project provides health maintenance and acute care services to homeless men, women, infants and children, and provides street outreach services at community locations where homeless individuals may congregate. Respite services are offered to homeless adults suffering from acute illness or injury who need a safe place to recover.

Work locations for these positions will include the Salvation Army's Harbor Lights facility, Catholic Charities' Branch III Drop-In Center, Sharing and Caring Hands, People Serving People and Mary's Place. Work hours for these part-time, benefit earning positions (between 56 and 72 hours per two-week pay period) are Monday through Friday and include working one or two evening clinics each week. Work hours will vary; clinic hours range from 7 AM to 8:30 PM. No weekend, overnight or holiday shifts are required.

Persons hired for these positions may also be asked to provide services at the Hennepin County Public Health Clinic, which provides confidential diagnosis and treatment of sexually transmitted infections (STIs), testing and counseling for HIV, treatment for tuberculosis and latent TB infection (LTBI), and health screening for newly arrived refugees.

Successful candidates must have a vehicle available for travel to local shelters and drop-in centers; some mileage and parking reimbursement will be provided by Hennepin County.

Primary duties and responsibilities include:
  • Perform initial and ongoing assessments of patient medical, physical and psychosocial status.
  • Triage patients who require physician supervision.
  • Obtain and evaluate patient health and medical history and document in the EPIC Electronic Health Record (EHR).
  • Perform physical examinations and conduct preventive screening procedures based on age and medical history; identify medical and health risks.
  • Complete accurate and timely documentation of observations, interventions, referral and follow-up; develop patient care plan.
  • Order appropriate therapeutic treatments; order, conduct and interpret diagnostic tests.
  • Prescribe, administer and dispense therapeutic devices and legend drugs.
  • Provide health education, counseling, consultation and advocacy based on needs identified in care plan.
  • Refer clients to other health professionals and community agencies.
  • Assist with ongoing medical and nursing staff teaching programs; initiate and participate in research studies.
  • Develop and maintain positive relationships with community service providers.
  • Serve as an active team member of a multi-disciplinary team by attending meetings; actively participating in program planning, development and evaluation; and assisting with reports and special projects.
  • Provide street outreach services for homeless people at community locations where homeless individuals may congregate.
  • Ideal Candidates must have:
  • Master's degree in nursing from an accredited school of nursing.
  • Possession of a valid Minnesota license as a Registered Nurse.
  • Current certification as a Nurse Practitioner.
  • Two or more years of recent experience as an Adult or Family Nurse Practitioner providing services to medically underserved populations.
  • Sensitivity to the needs and issues of diverse and disenfranchised individuals.
  • Knowledge of public health promotion.
  • Ability to work independently, counsel and instruct individuals in a state of stress, and act as an advocate.
  • Strong leadership, verbal and written communication skills.
  • Ability to establish and maintain effective relationships with internal and external partners.
  • Current CPR certification.
  • Ability to demonstrate the employee competencies.
