Under supervision and within State, Federal and County developed programs and policies, identifies client needs, creates and develops a foster care licensed home, and a foster care program to support quality foster homes. Implements, monitors and evaluates the foster care licensed home. Provide case management services for adoption/ guardianship, and interstate compact cases; identifies adoptive homes, create and develop home studies, and monitor adoptive placements. |

MINIMUM QUALIFICATIONS:
|
· Bachelor's degree in Social Work, Psychology, Sociology, or related field AND
· One year full-time professional or 2 years of para-professional experience in a social services agency
And
· Valid Driver's License

DUTIES & RESPONSIBILITIES:
|

FOSTER CARE
1. Effectively engage families and corporations to assess and evaluate foster care eligibility, strengths and needs of the provider.
2. Collaborate with agencies to determine policy and procedures as they relate to licensing rules and regulations.
3. Provide consultation, support and information to license holders and others regarding services so they conform to the needs of clients.
4. Evaluate, document, and monitor eligibility for licensure and effectiveness of services provided.
5. Maintain and apply knowledge of relevant policies, procedures, statutes and professional practice.
6. Complete agency, state, and federal required documentation as related to client/provider eligibility according to mandated timelines.
7. Close cases based on eligibility or client desire to discontinue services.
8. Act on behalf of the Commissioner of Human Services.
ADOPTION
1. Complete Inter-state Compact home study requests/ case supervision.
2. Complete adoption homestudies as needed.
3. Case manage adoption/ guardianship and/or interstate compact cases including court responsibilities and monthly contacts.
4. Team with the ongoing child protection worker in all pre-adoptive activity.
5. Provide consultation to workers, colleague and the State on adoption issues.
6. Complete necessary Adoption Assistance or MN Permanency Demonstration paperwork as needed.
7. Facilitate monthly adoption support group for both children and parents.

KNOWLEDGE, SKILLS & ABILITIES AND WORK ENVIRONMENT:
|
· Knowledge of human development, family dynamics and systems, including crisis management and cultural competency.
· Knowledge of the Human Services Licensing Act, Adoption rules and statutes, and DHS Rules that directly affect foster care homes and adoption.
· Knowledge of medical diagnoses, medications, basic medical terminology and basic safety concerns for recipients of home studies.
· Knowledge and understanding of developmental disabilities and senior issues.
· Comprehensive knowledge of computer software and information systems.
· Interviewing, negotiation, and listening skills.
· Ability to communicate effectively both orally and in writing.
· Ability to investigate licensing concerns and independently set a course for positive change.
WORK ENVIRONMENT: Work is performed in a County office building and on client home visits. Some travel is required. Lifting requirement of up to 20 pounds on an occasional basis. Exposure to inadequate lighting and intense noise may occur on client home visits. Equipment used may include, but is not limited to, computer, printer, telephone, calculator, and copy and fax machines.
