The Anoka County Community Health and Environmental Services Department is establishing a register to fill current and future vacancies for a Public Health Nurses - MA Waiver (i.e. full-time, part-time and temporary). This position will provide quality nursing care to eligible Anoka County residents by promoting health and preventing disease; restoring client functions; and promoting a high level of wellness and independence. The Public Health Nurse will provide assessment/Long Term Care Consultation to determine eligibility for MA Waiver programs. This position will also provide case management of individuals who are at risk for out-of home placement so that the appropriate services can be utilized to provide health care and promote continued residence in the community.
EXAMPLES OF ESSENTIAL DUTIES (ILLUSTRATIVE ONLY):
Provides Long Term Care Consultation and determines eligibility for various Waiver Program (i.e. Alternative Care (AC), Elderly Waiver (EW), Community Alternatives for Disabled Individuals (CADI) and the Community Alternatives for Care (CAC) programs); Develops and implements community care plans for clients remaining in their homes under these programs; Provides case management services for individuals receiving services through the waiver programs; Assists families and individuals to understand the implications of their medical diagnosis and assists them in carrying out the care and treatment as prescribed by their physician; Develops, organizes and maintains accurate records of care provided; Consults with other professionals relative to individual client problems; Participates in public health nursing meetings; Performs public information service for the agency. Actively participates in key roles for Emergency Prepardness activities/events.
REQUIREMENTS:
Bachelor's Degree from a school of nursing accredited by the National League of Nursing; current licensure as a Registered Nurse in the State of Minnesota, plus Public Health Nursing certification; one to three years of related experience in public health nursing or highly related area preferred; or equivalent combination of education and experience. Knowledge of: Public health nursing principles and practices; health and disease processes and nursing interventions; community resources available to the patient. Ability to: Work with clients and their families; meet and deal with the public; make decisions and exercise independent judgment; prepare and maintain accurate charts, records and reports; develop a plan of care for a client; determine when ancillary services to a patient are required; lift and carry with ease nursing equipment and supplies.
Skill in: Written and oral communications.
Must have own transportation for business use (e.g. home visits, attend meetings at various sites, etc.); a good driving record.
